Simply download the PDF file of the Request for Plating, fill out the form, then email or fax it back to them and they will return an estimate to you.

If you'd rather use your own company form, please be sure to include the following information:

• Date
• Your name, company name, address, and phone
• Purchase order number
• Part number
• Total number of pieces in the order indicating rack or barrel plate
• Certification information, if required
• Required processes
• Please include a print of the part indicating the material and final use
