I remember a company supplying a higher grade of bolt thinking it was better and they were sued a load of money as the application required a lower grade product as they had inspectors that checked fatigue issues on a regular basis.

This can be fatal. Failure of fasteners especially at high pressures, temperatures or high speed would be very dangerous for equipment as well as for users.

Material and Dimensions of bolts should be clearly specified, especially sometimes threaded length is not specified in machine bolts properly for partial threaded and straight shant bolts.

Quality is overall. What may seem to be a cheaper solution in purchasing may turn out to be more expensive when the shipping costs are calculated for shipments of bolts that are inferior. Returning shipments of bolts adds to everyone's bottom line. Failure of bolts cause increased liability. Always be certain the your supplier succeeds in quality control for every production.
