Sure. The drive components used were too small, belt tensions were too high for the belt specified, thermal expansion of the structure in long run systems, and failure to consider arresting mechanisms in case of catastrophic failure.

Buying - get multiple quotes, make sure they guarantee performance, design good loading points, use good idlers and bearings. Size motors to start under full load. Using - make sure they have made the unit robust and easy to maintain, keep an eye on the belt tracking and cleanliness.
