Do you know of any disastrous mistakes that occurred due to the incorrect usage of protective relays and monitoring relays?

1. Incorrect application of Residual Earth-fault Protection alone to Power transformers have resulted to serious damages due to improper monitoring of Neutral Current flow which is Disastrous to transformers in active service. 2. Residual Earth-fault Protection presently applied to Distribution Feeders has proved disastrous on High-impedance earth-faults, resulting to loss of lives and damage to valuable equipment for persistent uncleared faults.

Electric motors got burnt as a result of low standard of overload relay used and incorrect setting as a result of lack of or little knowledge of the relays.
