Guden Weld-On Bullet Hinges are the answer.  Each piece is a rounded barrel with a small edge to weld onto your frame and door without seeing a hinge leaf. It's 2-piece male and female design allows for lifting and removing the door. Each hinge set is supplied with a friction washer and can also be supplied with a grease fitting attachment.
