  1. Most of the case studies are based on real cases in a number of countries, but they have been changed sufficiently to avoid identification of the particular facilities, and have been simplified to clarify the points being illustrated.

  2. The worked examples are also simplified to illustrate the methods more clearly.

When you have studied this chapter, you will have a broader understanding of some of the practical applications of the philosophies and methods of risk and reliability management, and a better understanding of some of the applications of the calculation methods covered in earlier chapters.

14.1 CASE STUDY: LPG FACILITY RISK

14.1.1 Introduction

A provincial town had, in the past, been supplied with gas for domestic purposes from a coal gasification plant. Some years ago it had been closed down and replaced with a facility that used LPG. The LPG was delivered to the site by road tankers and stored on site. LPG was piped to an evaporator, and the vapor produced was diluted slightly with air to a level still above the upper flammable limit, but with combustion characteristics similar to the old coal gas. The LPG vapor/air mixture was stored in the town's gas holder, previously used for coal gas, and distributed to the users by means of the original coal-gas network of pipes.

The regulatory authority of the state in which the facility was located introduced legislation requiring all "hazardous" facilities to prepare emergency plans in collaboration with their local councils and emergency services.
