Appendix A: Root Cause Map Node Descriptions
Overview
This appendix is designed to help you use the Root Cause Map in a consistent manner. It contains a list of typical issues, typical recommendations, and examples for each node on the map. The appendix covers the nodes on the map from left to right and from top to bottom. To help you find information on a specific node, two node listings are provided.
The first list is organized by areas of the map. This listing is useful if you are interested in examining a particular portion of the map.
The second list provides an alphabetical listing of the nodes. This list can be used to locate information concerning a specific node.
A third index is actually contained on the Root Cause Map in Appendix B. On the map contained in Appendix B, each node has a number in the graphic following the text. The number in the graphic indicates the page number in Appendix A that corresponds to that node. By using this version of the map, a user can directly determine the appropriate page in the handbook without the need for a separate index.
